Middleton Physio provides evidence-based physiotherapy assessment, treatment and advice in-person and online. All care is delivered by a HCPC-registered physiotherapist and covered by CSP Professional Liability and Public Indemnity Insurance.

  1. Scope of Practice

The physiotherapist is trained to recognise presentations that fall outside their musculoskeletal (MSK) scope of practice. If your condition requires further medical evaluation or investigations (such as blood tests, X-rays, or MRI scans), you may be referred, with your consent, to your GP or another appropriate healthcare professional.

Middleton Physio does not provide emergency care. In an emergency, contact 999 or attend your nearest emergency department.

  1. Fees & Payment
  • Initial appointment per session (≈40 minutes): £65
  • Follow-up appointment per session (≈40 minutes): £65

Payment Terms:

  • Online: Payment in full prior to the session
  • In-person: Payment at the appointment (card or cash)

Discounts or promotions may be offered at the physiotherapist’s discretion. Prices may change with reasonable notice.

  1. Patient Eligibility
  • Services are for individuals aged 16 and over
  • Patients aged 16 and over may consent to their own treatment in accordance with UK law
  1. Provision of Information

Treatment relies on accurate information provided by you. You agree to:

  • Complete online questionnaires or follow-up questions honestly
  • Provide any information reasonably relevant to your condition or treatment

Required information should be submitted before your appointment as indicated in your confirmation email.

  1. Preparation for Assessment & Patient Rights

For effective assessment and treatment, clothing may need to be adjusted or removed. This will always be done with your consent. Please wear comfortable clothing that allows easy movement during your appointment. Ideal attire for upper-limb assessment is a sports top or vest and for assessments from the hip down, shorts are recommended.

You may:

  • Ask questions at any time
  • Request a chaperone
  • Pause or decline treatment
  • Request a copy of your records
  1. Cancellations & Missed Appointments
  • A 24-hour notice is required for cancellations to allow others to access the service.
  • Late cancellations or missed appointments incur the full session fee, except in emergencies.

  1. Liability

Middleton Physio provides care in line with professional standards. Every effort is made to provide safe and effective care. We are not liable for injury, worsening of your condition, or loss unless directly caused by our negligence, except as required by law.

  1. Confidentiality & Data Protection

Your information is protected under UK GDPR (2018) and professional confidentiality standards:

  • Records are securely stored in Cliniko (encrypted, 2FA)
  • No client data is stored locally
  • Information is shared only with your consent or when required by law
  1. Record Retention
  • Adults: minimum 8 years from last treatment
  • Minors: until patient reaches 25 years of age
  1. Complaints

Concerns should be raised with Calum Middleton at calum@middletonphysio.co.uk. If unresolved, you may escalate the complaint to professional regulatory bodies, including the HCPC and CSP.

  1. Clinical Audit
  • Treatment data may be used for ongoing audit or service improvement.
  • Any data used for audit or service improvement is anonymised.
  1. Photography & Video

With your consent, photos or videos may be taken for treatment records or shared for educational/promotional purposes. Consent can be withdrawn at any time.

  1. Communication

You may receive appointment reminders and care-related messages via SMS, email, or WhatsApp. These may include exercise links, follow-up advice, or messages and calls about booking or changing appointments. While care is taken to protect your privacy, these platforms are not fully secure and should not be used for urgent or emergency matters. Marketing messages can be opted out of at any time.

  1. Online Physiotherapy Services

To receive physiotherapy services online, you agree to:

  • Have a reliable internet connection and necessary equipment (microphone, headphones, camera)
  • Use Microsoft Teams, WhatsApp, or another agreed platform
  • Accept that Middleton Physio cannot be held responsible if technical issues affect the consultation

Use of Online Services Outside the UK:
Online physiotherapy services are intended for patients accessing care from within the UK. Use from outside the UK is at your own risk, and you are responsible for ensuring it is legal and appropriate in your location.

  1. Suitability and Termination of Services

Middleton Physio reserves the right to terminate any treatment, whether online or in-person, in the following circumstances:

  • We determine, after assessment, that the treatment is not appropriate or safe, including if your medical condition changes in a way that makes treatment unsafe.
  • There is non-compliance with treatment specifications or repeated missed appointments.
  • There is disruptive, disrespectful, or abusive behaviour.
  • Non-payment occurs, or other professional, legal, or regulatory reasons arise.

Where possible, we will provide an explanation and, where appropriate, signpost to alternative services.

  1. Outcomes
  • Every effort is made to provide effective care; however, as with any physiotherapy, outcomes cannot be guaranteed.
  • Online consultations are not a complete substitute for in-person appointments.
  • Treatments are recommended based on professional training, experience, and evidence.
  1. Changes to Terms & Privacy Policy

These Terms may be updated periodically. Updates will be posted with a new “Last Updated” date. Continued use of our services constitutes acceptance of changes.

  1. Consent Statement

By completing the Middleton Physio intake form, you acknowledge that you:

  • Understand risks and responsibilities of treatment
  • Agree to fees, payment, and cancellation policies
  • Consent to clinical data use for audit
  • Consent to photography/video as described
  • Consent to communications regarding care
  • Understand that outcomes cannot be guaranteed
  • May withdraw consent at any time
  • May ask questions, pause or decline treatment, and request a copy of your records
